► My mother died 14 days back she died at home of attack 15 minutes time i wascalling ambulance giving her water to drink hot coffee so i didnt know any doctor Societycalled police they took my mother to Sion hospitalwhere she was declared dead doctor did postmortem of body said cause of death heart attack .i creamated her they said show receipt to Bmc your ward they will issue death certificate.What should I do to get her Death certificate. I don't know how to apply for it .please explain

To apply for a death certificate in India, register the death within 21 days at your local municipality, Panchayat, or online via the Civil Registration System (CRS) portal. After your mother’s death at home and post mortem at Sion Hospital, BMC can issue the death certificate once the death is formally registered. You should first collect the hospital “Cause of Death” certificate (or post‑mortem sheet) and the crematorium receipt/certificate from the burial ground, along with your Aadhaar or ID and your mother’s ID or address proof (if available), and then visit the BMC ward office in your mother’s area (or in the ward where the hospital/cremation is located, if different) and ask for the death‑registration form (Form 2), fill it with your mother’s details, date of death, place of death, cause of death (“heart attack”), and your name as the informant, and submit it with the hospital and cremation documents and a small fee. If you prefer, you can also apply online through the BMC Death Certificate portal, enter the required details, and then print the application form and submit it at the same ward office. BMC usually issues the death certificate within a few days once the file is complete and the death has been recorded in its system. If the ward staff demand any extra document or delay, ask for a written list of requirements and, if necessary, approach the Health/Registration section of your BMC zone office or Citizen Helpline for help.

Due to her being deceased at home, there is a police report, which resulted in an investigation into the cause of death. Due to the autopsy being conducted, they have confirmed that your mother's death was due to cardiac arrest. This information will be included in the original registration of your mother's death with the centralised BMC database.
Once the hospital uploads all details to the centralized portal, it is forwarded for approval to the Medical Officer of Health, and the death certificate can then be issued by any Municipal Ward Office regardless of where the death occurred. This means you do not necessarily have to go back to Sion, your own local Ward Office can issue the certificate.
Here is exactly what you need to do. First, take the cremation receipt that the crematorium gave you and go to the BMC Ward Office for the ward in which your mother was a resident. This is the ward they told you to visit. When you go there, carry the following documents the cremation receipt, your mother's Aadhaar card or any address proof in her name, your own Aadhaar card as the applicant, and the postmortem report or hospital discharge/death summary from Sion Hospital if they gave you any paperwork. The cremation certificate from the crematorium is the key document needed to apply for the death certificate at the BMC Ward Office. At the Ward Office, go to the Birth and Death Registration section, tell them the date of death, hospital where postmortem was done, and crematorium details. They will search the centralized system for the registered death entry and issue the certificate.
Since the death was reported to the police and the hospital has conducted a postmortem, the death will already be in the system you are not starting from scratch, you are simply collecting the certificate that has been generated. As per the Right to Services Act, the Death Certificate should be issued within two days of applying. If there is any delay or the ward office says the record is not found, ask them to check under Sion Hospital's registrations and give them the date of death and your mother's full name. You can also apply online at portal.mcgm.gov.in by searching for the death record using the date and name and then submitting your application digitally if you prefer not to visit in person.

In Mumbai, death registration is handled by the Brihanmumbai Municipal Corporation (BMC). Since the hospital conducted the postmortem, they usually forward the death report directly to BMC. However, because you were told to approach your ward office, you should visit the BMC ward office of the area where the death occurred or where the hospital is located. Carry the following documents: (i) hospital papers or postmortem report (if given), (ii) cremation receipt from the crematorium, (iii) any ID proof of your mother (like Aadhaar), and (iv) your own ID proof.

 

At the ward office, ask for the Death Registration section. They will check if the death has already been registered based on the hospital’s report. If it is registered, you can apply immediately for certified copies of the death certificate by filling a simple form and paying a small fee. If, for any reason, the record is not yet entered, they will guide you to submit the documents so that registration can be completed first. Since the death happened recently (14 days), it is still within the normal reporting period, so the process should be straightforward without penalties.
